Pop up vaccine clinic for L3S Senior Residents

Date:   Monday to Friday, May 17 – May 21
Time:   Mon & Tues, May 17 & 18, 10:00 am – 6:00 pm
           Wed to Friday, May 19 to 21, 1:00 pm – 9:00 pm
Place: Islamic Society of Markham
           2900 Denison Street, Markham, ON L3S 4J8 (Denison & Middlefield)
Eligibility:
- Adult 60 and older (Born in or before 1961), live in L3S / L6G Postal Code
- A companion (aged 18 , does not need to reside in L3S / L6G Postal Code) accompanying the eligible senior for vaccination

The clinic is expanded to age 18 and (born in or before 2003) for L3S postal code.

Clinic is for first-dose appointment only. NO WALK-INS.

Carefirst vaccination site will go live on Tuesday, April 6, 2021 with details as follow:

Start date: April, 6 2021 (Tuesday)

Vaccination site opening hours: Monday to Friday 9:00 a.m. - 5:00 p.m.

Eligibility:

Residents of City of Scarborough: age 60 or over (Proof of age and address will be required on the day receiving the vaccine )

Residents of Toronto: age 50 or over with chronic conditions (Proof of age, address, and medication prescription will be required on the day receiving the vaccine)

Before the government’s online booking system available, booking can be done thru Carefirst with details as follow:

Please call 416- 646-5108 Friday, April 2 & Saturday, April 3, 9 am – 5 pm (we have staff on call to answer the phone call for the booking) We are now booking for April 6, 7 and 9.
